Job Summary As a Care Manager (RN), you will play a critical role in developing, assessing, and facilitating complex care management activities for members with primarily physical needs. You'll work closely with members, families, caregivers, and healthcare providers to create personalized care plans, identify barriers to care, and provide ongoing monitoring and support. Key Responsibilities Evaluate member needs, barriers to care, and social determinants of health, focusing on member priorities and recommending plans for optimal outcomes Develop and implement ongoing care plans, identifying providers, specialists, and community resources to address member unmet needs Coordinate between members, families, caregivers, and healthcare providers to ensure person-centered care services Provide ongoing monitoring of member status and progress towards care plan goals Collaborate with members, caregivers, and providers to revise or update care plans as necessary Facilitate care management and collaborate with providers or specialists to ensure timely access to needed care or services Perform telephonic, digital, home, or site outreach to assess member needs and collaborate with resources Collect, document, and maintain member information and care management activities to ensure regulatory compliance Provide education to members and families on disease processes, care gaps, healthcare provider instructions, and healthcare benefits Provide feedback to leadership on opportunities to improve care and quality delivery Essential Qualifications To succeed in this role, you'll need: A Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2-4 years of related experience in care management or a similar field RN - Registered Nurse - State Licensure and/or Compact State Licensure Strong clinical knowledge and experience working with diverse populations Excellent communication and interpersonal skills Ability to work independently and as part of a remote team Preferred Qualifications While not required, the following qualifications can enhance your application: Experience working with Medicaid or Medicare populations Knowledge of care management software and technology Certifications in care management, such as CCM or ACM Experience working in a remote or telephonic care management environment Skills and Competencies To excel as a Care Manager (RN), you'll need: Strong analytical and problem-solving skills Ability to prioritize and manage multiple cases simultaneously Effective communication and interpersonal skills Empathy and compassion when working with members and families Ability to work in a fast-paced, dynamic environment Career Growth Opportunities At Centene, we're committed to helping you grow your career.
