Key Responsibilities Provide skilled and compassionate nursing care to patients in their homes, focusing on their physical, emotional, and social needs. Assess patients' conditions, develop and implement care plans, and evaluate the effectiveness of care. Administer medications, treatments, and procedures as prescribed by physicians, and monitor patients' responses. Educate patients, families, and caregivers on self-care techniques, disease management, and health promotion. Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care. Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes. Participate in quality improvement initiatives and contribute to the development of policies and procedures. Requirements and Qualifications Essential Qualifications: Active LPN license in the state of Kentucky. At least one year of clinical experience as an LPN. CPR certification. Valid driver's license. Proficiency in technology, including smartphones and electronic medical records. Excellent communication and interpersonal skills. Preferred Qualifications: Experience in home healthcare or a similar setting. Knowledge of wound care, medication management, and disease-specific care. Certification in a specialized area, such as gerontology or pediatrics.
