Social Determinants of Health Community Strategist

CVS Health is seeking a Social Determinants of Health Community Strategist to join our team and drive our efforts in promoting health equity. In this role, you will lead our community outreach and partnership initiatives, working closely with local organizations and stakeholders to address social determinants of health and improve health outcomes for all. We are looking for a highly motivated and experienced individual who shares our commitment to making a positive impact on the health of our communities. If you have a strong understanding of social determinants of health and a track record of developing effective community strategies, we want to hear from you. Develop and implement community outreach and partnership initiatives to address social determinants of health. Collaborate with local organizations and stakeholders to identify health disparities and develop targeted strategies to promote health equity. Conduct research and analysis to identify key issues and trends related to social determinants of health. Develop and maintain relationships with community partners to ensure effective collaboration and maximize impact. Create and deliver presentations and trainings to educate stakeholders on social determinants of health and strategies for addressing them. Monitor and evaluate the effectiveness of community initiatives and make recommendations for improvement. Stay current on national and local policies and initiatives related to health equity and social determinants of health. Represent CVS Health at community events and meetings to raise awareness and advocate for health equity. Develop and manage budgets for community outreach and partnership initiatives. Collaborate with internal teams to integrate social determinants of health strategies into company-wide initiatives and programs. Foster a culture of diversity, inclusion, and cultural competency within the organization. Serve as a subject matter expert on social determinants of health for the organization and provide guidance and support to colleagues. Act as a liaison between CVS Health and community organizations, communicating our goals and initiatives and identifying opportunities for collaboration. Stay informed on best practices and emerging trends in addressing social determinants of health to continuously improve our approach.
