About the position Responsibilities • Plan and administer prescribed skilled physical therapy treatment and training for patients. • Test/screen the patient's physical strengths to assist in evaluating their level of function. • Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment. • Confer with physicians and clinical team members to obtain additional patient information. • Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aide staff. • Accurately document patients' care observations, interventions, and evaluations. • Report patient's progress to the physician, Clinical Manager, staff, patient, and family. Requirements • Degree from an accredited Physical Therapy Program (approved by the APTA). • Minimum of one year physical therapy experience preferred. • Current and unrestricted Physical Therapy licensure. • Current CPR certification. • Strong organizational and communication skills. Nice-to-haves • Home Health experience a plus. • Compact Physical Therapy License preferred.
