Job Summary As a Home Health Registered Nurse at CenterWell Home Health, you will be responsible for providing hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under the supervision of our Clinical Manager. You will confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment, and revise the plan in consultation with the physician based on ongoing assessments and as required by policy/regulation. Key Responsibilities Provide admission, case management, and follow-up skilled nursing visits for home health patients, ensuring that each patient receives comprehensive and individualized care. Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager to ensure that patient needs are met and outcomes are achieved. Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment, ensuring that the care plan is tailored to the patient's unique needs and goals. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision, to ensure that patients receive the highest quality care. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation, ensuring that the care plan remains current and effective. Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers), to ensure that patients receive comprehensive and coordinated care. Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis, ensuring that all stakeholders are informed and up-to-date on patient status. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems, promoting patient engagement and empowerment. Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, and provide written personal care instructions/care plan that reflect current plan of care, ensuring that all team members are working together to deliver high-quality care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility, ensuring that all documentation is accurate, complete, and timely. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation, to ensure that patients receive seamless transitions of care. Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes, ensuring that all documentation is accurate, complete, and timely. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care, promoting collaboration and communication among the healthcare team. Essential Qualifications Diploma, Associate, or Bachelor's Degree in Nursing, providing a strong foundation in nursing principles and practices. Minimum of one year nursing experience preferred, with a strong background in med-surg, ICU, ER, or acute care, demonstrating a ability to work in fast-paced and dynamic environments. Home Health experience is a plus, with a understanding of the unique challenges and opportunities of delivering care in the home setting. Current and unrestricted Registered Nurse licensure, ensuring that you are qualified and authorized to practice as a Registered Nurse in the state of Georgia. Current CPR certification, demonstrating a commitment to providing high-quality and safe care. Strong organizational and communication skills, with the ability to prioritize tasks, manage time effectively, and communicate clearly and effectively with patients, families, and healthcare teams. Valid driver's license, auto insurance, and reliable transportation, enabling you to travel to patient homes and other locations as needed. Preferred Qualifications Bachelor's Degree in Nursing, providing advanced knowledge and skills in nursing principles and practices. Two or more years of nursing experience, with a strong background in home health care, demonstrating a deep understanding of the unique challenges and opportunities of delivering care in the home setting. Experience with electronic medical records (EMRs) and other healthcare technology, demonstrating a ability to leverage technology to improve patient care and outcomes. Certification in a specialty area, such as gerontology or pediatrics, demonstrating a commitment to providing high-quality and specialized care. Bilingual or multilingual skills, enabling you to communicate effectively with patients and families from diverse backgrounds.
