Job Summary and Qualifications As a PRN Home Health Registered Nurse, you will play a vital role in our team, providing coordinated skilled nursing care to patients of all age groups in their homes. You will work collaboratively with our interdisciplinary team to establish and achieve patient goals, maintain high-quality patient care, and ensure seamless communication with patients, families, and healthcare providers. Key Responsibilities: Assess home care patients, identifying physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, team reports, call-in logs, and on-site evaluations. Complete OASIS, assessment, and visit paperwork according to agency policy. Ensure clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members, and patient/family/caregiver. Communicate significant findings, problems, and changes to Clinical Manager and physician, and document all findings, communications, and appropriate interventions. Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided. Participate in on-call rotation and emergency call according to agency policy. Essential Qualifications: Current Registered Nurse licensure in the USA. Basic Cardiac Life Support (BCLS) certification, obtainable within 30 days of employment start date. Associate Degree or Registered Nurse Diploma. Driver's license. At least 1 year of experience in home health care or a related field. Preferred Qualifications: Experience working with patients with diverse needs and backgrounds. Strong communication and interpersonal skills. Ability to work independently and as part of an interdisciplinary team. Familiarity with electronic medical records and OASIS documentation.
