Job Summary We're seeking a full-time Registered Nurse to provide skilled nursing care to patients in their homes. As a Home Health Registered Nurse, you'll be responsible for assessing patient needs, developing and implementing care plans, and providing ongoing care and case management. This is a unique opportunity to work in a remote setting, with the flexibility to create your own schedule and work from home. Key Responsibilities Provide admission, case management, and follow-up skilled nursing visits for home health patients Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager Confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment Provide hands-on care, management, and evaluation of the care plan, and teach patients in accordance with physician orders, under Clinical Manager's supervision Revise plans in consultation with physicians based on ongoing assessments and as required by policy/regulation Coordinate care with various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers Monitor documentation completion by home health aides/personal care workers as part of supervisory/leadership responsibilities Discharge patients after consultation with physicians and Clinical Managers, preparing and completing needed clinical documentation Prepare medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes Requirements and Qualifications To be successful in this role, you'll need: A Diploma, Associate, or Bachelor's Degree in Nursing Minimum of one year of nursing experience, with strong med surg, ICU, ER, or acute experience Home Health experience a plus Current and unrestricted Registered Nurse licensure Current CPR certification Strong organizational and communication skills Valid driver's license, auto insurance, and reliable transportation Preferred Qualifications We'd love to see: Bachelor's Degree in Nursing Experience working in home health care Certification in a specialized area of nursing practice (e.g., wound care, palliative care) Skills and Competencies To excel in this role, you'll need: Excellent communication and interpersonal skills Strong critical thinking and problem-solving abilities Ability to work independently and as part of a team Compassion, empathy, and respect for patients and families Strong organizational and time management skills Ability to prioritize tasks and manage multiple responsibilities Career Growth Opportunities and Learning Benefits At CenterWell Home Health, we're committed to supporting the professional growth and development of our nurses.
