Key Responsibilities: Patient Care Coordination: Provide admission, case management, and follow-up skilled nursing visits for home health patients, ensuring comprehensive care and support. Care Planning and Implementation: Confer with physicians to develop and revise individualized plans of treatment, based on patient assessments and physician orders. Patient and Family Education: Teach patients and their families about their conditions, treatments, and self-care techniques to promote optimal health and well-being. Collaboration and Communication: Work closely with healthcare professionals, including physicians, Physical Therapists, Occupational Therapists, and Home Health Aides, to coordinate care and ensure seamless transitions. Documentation and Reporting: Prepare and maintain accurate, up-to-date medical documentation, including patient records, visit reports, and care plan updates. Leadership and Supervision: Oversee and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing guidance and support as needed. Discharge Planning: Coordinate patient discharge, in consultation with physicians and Clinical Managers, and ensure that all necessary clinical documentation is completed. Requirements and Qualifications: Education: Diploma, Associate, or Bachelor's Degree in Nursing from an accredited institution. Experience: A minimum of one year of nursing experience, preferably in a med-surg, ICU, ER, or acute care setting; home health experience is a plus. Licensure and Certification: Current, unrestricted Registered Nurse licensure in the state of Alabama; current CPR certification. Skills and Competencies: Strong organizational, communication, and interpersonal skills; ability to work independently and as part of a team; valid driver's license, auto insurance, and reliable transportation.
