Job Summary As a Medical Social Worker in our Home Health Per Diem team, you will assess patients' social and emotional states, develop personalized plans of care, and provide direction and supervision to Social Worker Assistants as needed. You will work closely with our interdisciplinary care team to identify and address patients' emotional and social needs, and participate in discharge planning and care coordination activities. Key Responsibilities Patient Assessment: Evaluate patients' social and emotional states, including their relationships, financial resources, and community support systems. Plan of Care Development: Develop personalized plans of care that address patients' emotional and social needs, and collaborate with the interdisciplinary care team to implement these plans. Community Resource Coordination: Identify and coordinate community resources to support patients' needs, such as housing, shelter, funeral arrangements, and financial assistance. Patient and Family Education: Provide patients and families with education and support to help them cope with social and emotional responses to illness or injury. Discharge Planning: Participate in discharge planning and care coordination activities to ensure smooth transitions to other care settings. Collaboration and Communication: Collaborate with physicians, nurses, and other healthcare professionals to understand the social and emotional factors impacting patients' health, and communicate effectively with patients, families, and healthcare teams. Requirements and Qualifications Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education. Licensure: Social Worker licensure in the state of practice, if required by state law or regulation. Experience: Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice. Skills: Excellent oral and written communication and interpersonal skills, with the ability to read, write, and speak fluent English. Certifications: Current CPR certification and valid driver's license, auto insurance, and reliable transportation. Preferred Qualifications Experience with Discharge Planning: Knowledge of and experience with discharge planning needs, and ability to obtain community resources. Medication Knowledge: Knowledge of medications and their correct administration. Organizational Skills: Ability to organize tasks, develop action plans, set priorities, and function under stressful situations. Flexibility: Ability to be flexible in work hours and travel locally.
