Role Overview As a Nurse Practitioner or Physician Assistant with our REACH Program, you will play a vital role in delivering high-quality, patient-centered care to our geriatric patients in the comfort of their own homes. You will work collaboratively with our team of healthcare professionals to develop and implement comprehensive care plans, addressing the unique needs of each patient. Your expertise and compassion will be essential in providing routine physical examinations, formulating medical, educational, and psychosocial plans, and identifying problems outside of your scope of practice to refer appropriately to physicians. Key Responsibilities Conduct routine physical examinations and develop comprehensive care plans for geriatric patients in the home care setting. Collaborate with physicians and other healthcare professionals to ensure seamless continuity of care and address patient needs. Provide education and support to patients and their families, promoting health literacy and self-management of chronic conditions. Identify and address psychosocial needs, referring patients to appropriate resources and services as needed. Develop and maintain accurate and comprehensive patient records, utilizing our state-of-the-art clinical and informatics technology, EPIC. Essential Qualifications Licensure: Unrestricted State of Connecticut Medical License (or eligible) as a Nurse Practitioner or Physician Assistant. Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA, with a strong foundation in geriatric care. Experience: At least two years of post-graduate experience as a Nurse Practitioner or Physician Assistant, with a preference for experience in geriatric care or home care settings. Preferred Qualifications Certification: Board certification as a Nurse Practitioner or Physician Assistant, with specialization in gerontology or a related field. Education: Master's degree in Nursing or a related field, with a focus on gerontology or adult-gerontology primary care. Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
