About the Role As a Home Health Aide with Community LIFE, you will play a vital role in providing essential care and support to our program participants, assisting them with daily living activities, and enabling them to maintain their independence and quality of life. Key Responsibilities: Provide Home Health Aide assistance with Activities of Daily Living (ADLs) to program participants in their homes, including bathing, showering, toileting, dressing, meal preparation, feeding, and light cleaning Occasionally work in the day center as needed, providing support and care to program participants in a community-based setting Develop and maintain positive relationships with program participants, their families, and other healthcare professionals, ensuring a collaborative and patient-centered approach to care Maintain accurate and up-to-date records of patient care and progress, ensuring that all documentation is complete, accurate, and compliant with regulatory requirements Participate in ongoing training and education, staying up-to-date with best practices and industry developments, and maintaining the skills and knowledge required to deliver exceptional patient care Requirements and Qualifications To be successful in this role, you will need to possess a combination of education, experience, and skills that demonstrate your ability to provide high-quality patient care. The essential requirements for this role include: A High School Diploma or GED Minimum of 1 year (12 months) of experience providing caregiving services for the frail or elderly Valid PA Driver’s license and reliable vehicle Ability to work independently and as part of a team, with a strong commitment to delivering exceptional patient care Excellent communication and interpersonal skills, with the ability to develop positive relationships with patients, families, and healthcare professionals Preferred Qualifications: Previous experience working in home health care or a related field Certification as a Home Health Aide or a related credential Experience working with patients with complex medical needs or disabilities Ability to speak multiple languages, with a strong understanding of cultural diversity and sensitivity Skills and Competencies To be successful in this role, you will need to possess a range of skills and competencies that demonstrate your ability to provide high-quality patient care.
