Job Overview We're seeking a skilled and dedicated Inpatient Care Coordinator who is a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) to join our team remotely in Nyack, NY. As a Skilled Inpatient Care Coordinator, you will be the crucial link between patients and healthcare personnel, ensuring smooth transitions of care. Primary Responsibilities: Perform Skilled Nursing Facility (SNF) assessments on patients using your clinical skills and CMS criteria upon admission and throughout their stay. Review Length of Stay (LOS) targets, outcomes, and discharge plans with healthcare providers and families. Conduct SNF concurrent reviews and update authorizations in a timely manner. Collaborate with patients' healthcare teams to establish optimal discharge plans. Monitor patients' progress toward discharge goals and assist in resolving any barriers. Participate in weekly SNF Rounds, providing accurate and up-to-date information. Ensure appropriate referrals are made to Health Plans, High-Risk Case Managers, and community-based services. Engage with patients, families, and caregivers through regular communication. Attend patient/family care conferences and assess patients' continued appropriateness for SNF settings according to CMS criteria. Coordinate peer-to-peer reviews with Medical Directors and support new delegated contract start-ups. Manage your caseload efficiently, utilizing time management skills and entering timely and accurate documentation. Review reports and dashboards with the Clinical Team Manager to identify opportunities for improvement. Adhere to organizational policies, maintain confidentiality of PHI, and stay updated on federal and state regulatory policies. Qualifications and Requirements Required Qualifications: Active, unrestricted registered clinical license in New York as an RN, PT, OT, or SLP. 3+ years of clinical experience in your respective field. Proficiency in Microsoft Office (Outlook, Word, Excel, and Teams). Reliable transportation to visit facilities throughout Rockland County (minimum twice a week). A dedicated, distraction-free home office space with high-speed internet access. Preferred Qualifications: Case Management experience, preferably with a geriatric population. Background in patient education, rehabilitation, or home health nursing. Understanding of CMS, Medicare, and Medicaid guidelines. Detail-oriented with the ability to prioritize multiple tasks and demonstrate excellent interpersonal and communication skills. Proven problem-solving, conflict resolution, and decision-making skills. The hourly range for this role is $33.75 to $66.25 per hour, based on factors such as local labor markets, education, and work experience.
