About the Role: LVN/LPN Care Management Specialist As a Remote LVN/LPN Care Management Specialist, you'll be an integral part of our Care Management team, focusing on the complex needs of our dual-eligible population. Under the guidance of a Registered/Licensed RN, you'll coordinate care for members experiencing significant health changes, ensuring seamless transitions between care settings. Your compassionate attention and excellent communication skills will facilitate holistic assessments, identify needs for clinical resources, and drive goal attainment. Key Responsibilities: Complete outreach cadence calls and post-discharge questionnaires within compliance-driven timelines, ensuring timely support for members. Utilize weekly and daily reporting to identify utilization trends, driving efforts to reduce Emergency Department Utilization and 30-day hospital readmissions. Follow members throughout their care continuum, from inpatient hospital stays to post-discharge recovery, ensuring a return to prior levels of functioning in the community. Facilitate Interdisciplinary Care Team Meetings with Social Services, Care Management, PCPs, and other key stakeholders to discuss service needs and support safe transitions. Collaborate with the care team to ensure optimal wellness, addressing the full continuum of members' healthcare and social determinant needs. Essential Qualifications: 2+ years of LPN or LVN nursing experience, with a strong background in care coordination or a related field. Active and unrestricted compact LPN or LVN licensure in your state of residence, ensuring compliance with regulatory requirements. Preferred Qualifications: Bilingual skills (Spanish) are highly desirable, enabling you to support a diverse member population. Discharge planning experience, with a deep understanding of the complexities involved in care transitions. Advanced proficiency in Microsoft Word, Excel, and Outlook, with the ability to adapt to new technologies and systems. Self-motivated, energetic, and detail-oriented, with excellent organizational skills and the ability to multitask in a fast-paced environment. Effective verbal and written communication skills, with the ability to craft clear, concise messages for diverse audiences.
