TFI Family Services is hiring a full-time Aftercare Case Manager to join our team!

What You?ll Need
? A bachelor?s degree in human services.
? A valid driver?s license.
? A willingness to learn?we?ll provide all the training you need!
A Day in the Life

As an Aftercare Case Manager, you?ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You?ll:
? Focus on risk management and help families maintain stability during the aftercare period.
? Provide crisis intervention and work with public and private agencies offering supportive services.
? Maintain accurate documentation and reports.
? Coordinate community and educational services for families.
? Meet with families in-person to support reunification and resolve any concerns.
