About the Role: The Home Health Nursing- Specialty Infusion position is essential for providing personalized, one-on-one care to patients in their homes or designated care facilities. The primary goal is to ensure the health, safety, and comfort of patients while promoting their independence and quality of life. Nurses in this role will assess patient needs, develop individualized care plans, and implement medical interventions as necessary. This position requires strong communication skills to effectively collaborate with patients, families, and other healthcare professionals. Ultimately, the Home Health Nurse plays a critical role in enhancing the overall well-being of patients through compassionate and skilled nursing care. Minimum Qualifications: • Valid nursing license (RN or LPN) in the state of practice. • Completion of an accredited nursing program. • Current CPR and First Aid certification. Preferred Qualifications: • Experience in home health care or home health- specialty infusion nursing. Responsibilities: • Conduct comprehensive assessments of patients' health status and needs. • Develop and implement personalized care plans in collaboration with patients and their families. • Administer medications and treatments as prescribed by physicians. • Monitor patients' vital signs and report any changes to the healthcare team. • Provide education and support to patients and families regarding health management and care procedures.
