About the Role: Hospice Patient Care Coordinator - Clinical Operations Specialist We're seeking a compassionate and clinically skilled Hospice Patient Care Coordinator to join our team in a remote capacity. As a key member of our clinical operations team, you'll provide critical support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, safe hospice care services. Key Responsibilities: Provide clinical and management support to the Clinical Nursing Supervisor, ensuring the day-to-day operation of the Hospice program. Assist with admissions, nursing visits, and staffing as needed, to ensure seamless delivery of care. Coordinate care with clients, families, caregivers, physicians, and other service providers to ensure comprehensive and patient-centered care. Monitor patient records to ensure compliance with documentation requirements and program-specific rules. Participate in quality assurance activities, including audits and data collection, to drive continuous improvement. Provide orientation, training, and in-services to new hires and the care team, to ensure the quality and appropriateness of care. Perform comprehensive nursing care to patients, including skilled nursing visits, coordination of care, and establishing plans of care. Maintain accurate and thorough documentation, including admission through discharge, physician's orders, and changes in condition. Requirements and Qualifications: Graduate of an accredited school of nursing. Current Registered Nurse licensure (in good standing) in the state(s) of practice. Current CPR certification. Valid driver's license and automobile insurance. Three (3) years of clinical experience, including: Two (2) years of hospice experience. One (1) year of supervisory experience in a clinical setting. Business coursework (business, finance, accounting, or human resources) preferred. Skills and Competencies: Passion and commitment to clinical excellence, quality, and integrity. Excellent interpersonal communication and patient/family engagement skills. Ability to work collaboratively as a team member, with a positive attitude and flexibility. Strong clinical documentation skills, with the ability to educate and inspire others. High energy, focus, work ethic, motivation, and commitment to lifelong learning and growth. Proficiency with EHR systems and their implementation/optimization in complex clinical settings. Knowledge and comfort level working with technology and enterprise-wide support systems and tools.
