Role Overview The Transition of Care Coordinator will be responsible for using advanced clinical judgment and decision-making skills to work with patients, families, and healthcare providers to ensure that patients receive the care and support they need as they transition from one healthcare setting to another. This will involve conducting assessments, developing care plans, coordinating services, and providing education and support to patients and their families. Key Responsibilities Conduct assessments of patients' needs and develop care plans to ensure seamless transitions between healthcare settings Coordinate services with healthcare providers, patients, and families to ensure that patients receive the care and support they need Provide education and support to patients and their families on self-care, medication management, and other topics related to their care Use clinical judgment and decision-making skills to identify and address potential barriers to care and develop strategies to overcome them Collaborate with healthcare providers and other stakeholders to ensure that patients receive high-quality, patient-centered care Use electronic health records and other technology to document patient information and communicate with healthcare providers Participate in quality improvement initiatives and contribute to the development of policies and procedures to improve patient care Essential Qualifications To be considered for this role, you will need to have: A Bachelor's degree in a related field, such as nursing, social work, or healthcare administration 3+ years of direct clinical practice experience, preferably in a hospital or alternative care setting Current licensure as a mental health professional in the state of Oklahoma (LPC, LMFT, LBP, or LCSW) Strong transportation and willingness to travel in-state up to 50% of the time Proficiency in MS Office Suite applications, including Word, Excel, and Outlook Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers Preferred Qualifications While not required, the following qualifications are preferred: Experience with case management and discharge planning Emergency intervention skills Managed care/ utilization review experience Experience working with Native American/Native Alaskan/Local American populations Skills and Competencies To be successful in this role, you will need to have: Strong clinical judgment and decision-making skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced and dynamic environment Strong organizational and time management skills Ability to use electronic health records and other technology to document patient information and communicate with healthcare providers Strong problem-solving and critical thinking skills Career Growth Opportunities and Learning Benefits Aetna is committed to the growth and development of its employees.
