Key Responsibilities Conduct comprehensive assessments of patients' needs and eligibility using care management tools and data/information audit Develop and implement personalized care plans to address patients' unique needs and goals Facilitate and coordinate care plan activities, monitor progress, and make adjustments as needed Collaborate with healthcare professionals, including caseworkers, managers, and clinical directors, to overcome barriers and achieve optimal results Identify high-risk factors and service needs that may impact patient outcomes and refer to clinical case management or crisis intervention as necessary Provide education, support, and empowerment to patients to make informed decisions about their healthcare and lifestyle choices Maintain accurate and timely documentation of patient interactions, care plans, and outcomes using case management and quality management processes Essential Qualifications 2+ years of experience in behavioral health or social services 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook) Bachelor's degree in behavioral health, human services, or a related field (psychology, social work, marriage and family therapy, counseling) or equivalent experience Preferred Qualifications Case management and discharge planning experience Managed Care experience Advanced degree in a related field (master's or non-licensed master's level clinician) Skill and Competency Requirements To succeed in this role, you will need to possess excellent communication, interpersonal, and problem-solving skills. You should be able to work independently and as part of a team, with a strong ability to prioritize tasks, manage multiple projects, and meet deadlines.
