About the Role: CAP RN/FT - Brunswick & Surrounding Counties We're seeking an experienced and compassionate Registered Nurse to join our team as a full-time Community Alternatives Program (CAP) RN. This remote work opportunity offers a flexible schedule, allowing you to make a meaningful impact in the lives of medically fragile children and their families. As a CAP RN, you'll play a vital role in providing basic case management services, coordinating with interdisciplinary teams, and ensuring quality care for those we serve. Key Responsibilities Establish client eligibility for services, obtain medical histories, and perform initial assessments to develop personalized care plans. Coordinate services with interdisciplinary teams, including Social Workers and other healthcare professionals, to ensure comprehensive care. Conduct interviews with clients, families, and healthcare providers to gather information for social histories and individual plans of care. Evaluate and document client programs, making adjustments as needed to ensure quality care and support. Provide basic case management services, serving as the lead member of a Social Worker/RN interdisciplinary team. Work closely with clients, families, and community agencies to identify and address needs, providing referrals to appropriate programs and services. Requirements and Qualifications To succeed in this role, you'll need: Graduation from an accredited School of Nursing, with a current RN certification. One year of nursing experience preferred, although not required. Thorough knowledge of professional nursing theory and practice, with the ability to direct and carry out prescribed medical treatments and therapeutic programs. A valid NC driver's license and a clean driving record (this is a condition of employment). Personal auto liability insurance if transporting clients in private/personal vehicles. Pre-Employment Screening As part of our hiring process, you'll be required to undergo pre-employment screening, which includes: Criminal background check. Name checks in registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities). Drug testing. Education verification and credentialing based on position requirements. Proof of employment history or references (if required). Proof of driver's license, driver's insurance, and vehicle (if required for providing transportation).
