Job Overview We are seeking an experienced and compassionate Hospice Patient Care Coordinator to join our team in Bloxom, VA. As a Patient Care Coordinator, you will provide clinical and management support to the Clinical Nursing Supervisor, ensuring the quality and safe delivery of hospice care services. You will be responsible for the day-to-day clinical operation of the Hospice program, assisting with staffing and scheduling, and providing direct patient care as needed. Key Responsibilities Assist the Clinical Nursing Supervisor in ensuring quality and safe delivery of hospice care services Provide direct patient care, including skilled nursing visits, coordination of care, and establishing plans of care Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers Assist in monitoring patient records to ensure compliance with documentation requirements and program-specific rules Participate in the QA process of medical records and assist in the completion of annual competencies and on-site visits for clinical staff Provide orientation, training, or in-services to new hires and the care team to assure the quality and appropriateness of care provided Assist in the completion of monthly and quarterly audits and facilitate the audit process for clinical and billing purposes Essential Qualifications Graduate of an accredited school of nursing Current Registered Nurse licensure (in good standing) in the state(s) of practice Current CPR certification Valid driver's license and automobile insurance Three (3) years of clinical experience, including two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting Preferred Qualifications Business coursework (business, finance, accounting, or human resources) in addition to a nursing license Experience with EHR systems and their implementation/optimization in complex clinical settings Proficiency with federal, state, and governing agency regulations Strong ability to be the public face of the organization and serve as an advocate for Home Healthcare, Home Hospice, and other service lines Skills and Competencies Passion and commitment to clinical excellence, quality, and integrity Positive attitude and ability to fit in as a collaborative team member for patient care Excellent interpersonal communication and patient/family engagement skills Commitment to clinical documentation excellence and quality, along with the ability to educate and inspire others High energy, focus, work ethic, motivation, and commitment to lifelong learning and growth Flexibility with diverse clients, cultures, and environments, allowing you to provide excellent care Career Growth Opportunities and Learning Benefits At Intrepid USA, we are committed to the growth and development of our team members.
