As a Hospice RN, you will be responsible for delivering professional nursing care to our patients in accordance with the established plan of care. You will lead by example, providing quality care consistent with all relevant professional standards and state and federal requirements. Your role will involve performing comprehensive subjective and objective assessments, formulating individualized plans of care, and providing skilled interventions aimed at achieving realistic outcomes. Key Responsibilities As a Hospice RN, your key responsibilities will include: Performing comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters Ensuring compliance with federal/state regulations governing hospice care services by following policy and procedure consistent with agency standards Formulating individualized plans of care that incorporate analysis of assessment data; modifying and updating as needed at a minimum of every two weeks on assigned patients Providing skilled interventions aimed at achieving realistic outcomes within a specified time period Assuring thorough and accurate documentation with all clinical data, including but not limited to: admission through discharge, physician's orders, skilled nursing visits, and changes in conditions Consistently demonstrating competency with technical nursing skills according to the legal scope of practice Implementing teaching specific to patient/family needs and evaluating and modifying as needed Performing clinical chart record reviews as requested Performing on-call responsibilities as assigned Performing patient visits in an efficient manner, completing and submitting all documentation timely and per branch policy Essential Qualifications To be successful in this role, you will need: A current RN license in the state of Missouri A minimum of 1 year of experience in hospice care or a related field Strong assessment and clinical skills Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Ability to maintain confidentiality and handle sensitive information Preferred Qualifications While not essential, the following qualifications are preferred: Experience with electronic medical records (EMRs) Certification in hospice and palliative care (CHPLN or CHPN) Bachelor's degree in nursing or a related field Experience with quality improvement initiatives Career Growth Opportunities and Learning Benefits At Intrepid USA, we are committed to the growth and development of our team members.
