Your time off is really yours." - Amedisys caregiver Job Summary and Responsibilities As a Hospice Care Case Manager, you will play a vital role in delivering high-quality, patient-centered care to individuals with terminal illnesses. Your primary responsibilities will include: Performing patient assessments and collaborating with the care team to develop and implement a plan of care Making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so Promoting patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques Supervising LPNs and HHAs to ensure high-quality care delivery Completing documentation timely and accurately Regularly communicating patient progress to the clinical manager and care team Planning and providing staff education to ensure ongoing professional development Performing on-call responsibilities and providing on-call services to patients/families as assigned Participating in clinical development and continuing education programs to stay current with industry best practices Qualifications and Requirements To be considered for this exciting opportunity, you must possess the following qualifications: Current RN license , specific to the state(s) you are assigned to work Current CPR certification Valid driver's license , reliable transportation, and liability insurance One (1+) year of clinical experience as a Registered Nurse (RN); if less than 1 year of clinical experience, candidate must be approved by VP Clinical* Note: If less than 6 months of clinical experience as a RN, candidate must participate in RN Intern program What We Offer: Competitive Compensation and Comprehensive Benefits At Amedisys, we understand the importance of offering competitive compensation and comprehensive benefits to our team members.
