Key Responsibilities: Delivering Exceptional Patient Care Verify the plan of care with the RN prior to each visit, ensuring that patient needs are met and care is coordinated. Collaborate with physicians and RNs to ensure patient needs are being met, notifying them promptly of any changes in patient condition. Teach patients and caregivers self-care techniques, promoting independence and confidence in managing their healthcare. Check vital signs and review medications, performing medication reconciliation at each visit to ensure accuracy and safety. Perform specialized procedures ordered by physicians, such as foley catheter changes, using aseptic technique to minimize risk. Prepare equipment and materials for treatments, maintaining a safe and efficient care environment. Complete documentation timely and accurately, ensuring that patient records are up-to-date and reflect the care provided. Qualifications: Joining Our Team To be considered for this role, you must have: One year of clinical LPN experience, demonstrating your ability to provide high-quality patient care. A current LPN license, specific to the state(s) you are assigned to work, ensuring that you're qualified to practice in your assigned location. Current CPR certification, reflecting your commitment to staying up-to-date with the latest life-saving techniques. A valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient visits safely and efficiently. Preferred qualifications include: Experience working in home health or a related field, demonstrating your understanding of the unique challenges and rewards of home-based care. Strong communication and interpersonal skills, enabling you to build strong relationships with patients, caregivers, and healthcare professionals. Compensation and Benefits: A Comprehensive Package We offer a competitive salary, ranging from $23.00 to $30.00 per hour, depending on your location, experience, and qualifications.
