Key Responsibilities Conduct thorough assessments of individuals' needs and eligibility using case management tools and data/information review Develop and implement care plans, identifying high-risk factors and service needs that may impact individual outcomes Facilitate and coordinate assigned care plan activities, monitoring progress and making adjustments as needed Collaborate with caseworkers, supervisors, Clinical Directors, and other healthcare programs to overcome barriers and achieve optimal results Identify and escalate quality of care issues through established channels, using influencing and motivational interviewing skills to ensure maximum individual engagement and promote lifestyle/behavior changes Provide education, information, and support to empower individuals to make informed decisions about their healthcare and lifestyle choices Assist individuals in actively and knowledgeably participating in their healthcare direction with their provider Use case management and quality management processes in compliance with regulatory and accreditation standards, company policies, and procedures Essential Qualifications 2+ years of experience in behavioral health or social services 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook) Bachelor's degree or non-licensed master's level clinician with a degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) or equivalent experience Preferred Qualifications Case management and discharge planning experience Managed Care experience Career Growth and Development Opportunities At arenaflex, we are committed to supporting the growth and development of our employees.
