Cardinal McCloskey Community Services is dedicated to protecting and empowering at-risk children and families. They are seeking a Case Planner to work with families at risk of foster care placement, guiding them through treatment and developing individualized service plans. Responsibilities Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization Conduct comprehensive assessments and develop individualized service plans Coordinate multidisciplinary interventions tailored to each family's unique needs Build meaningful relationships with families over an average case duration of 12 months Collaborate with internal and external providers to ensure safety, stability, and well-being Skills BSW or bachelor's degree in human services Some knowledge of the child welfare system Fluency in English and Spanish a Plus Citywide travel required Benefits Medical, Dental & Vision Insurance 403(b) Retirement Plan Generous Paid Time Off Flexible Work Schedules Company Overview CMCS provides services to over 2,000 children in foster care programs, preschool education to over 4,000 three and four-year-olds children.
