Remote LCSW or LPC JOB SUMMARY: The LCSW or LPC of the outpatient psychiatric program is responsible for the performance of a variety of clinical functions, while also providing direct clinical supervision for specified aspects of overall treatment programs. The Licensed Clinical Social Worker reports directly to the Director. Location: Telehealth Hours: P/T, up to 20 hours per week (weekdays) Compensation: up to $39 per hour ESSENTIAL FUNCTIONS: • Evidence knowledge of the Corporate Compliance, Privacy (HIPAA), Confidentiality of Information, and Ethics policy and adheres to the guidelines of the policy relative to position and licensing standards and other regulations and governing agencies of the organization. • Conducts patient assessments. • Completes patient bio-psychosocial, to include diagnostic narrative summary. • Conducts group and individual therapy sessions for the Program. • Completes inpatient consults as requested. • Responsible for adherence to the policies and procedures of the Program. • Assists the multi-disciplinary team with completion of patient master treatment plan. • Develops discharge instructions upon completion of outpatient services. • Performs all other duties as requested by Program. • Assesses home environment and offers assistance in resolving non-medical issues that impact on the patient's care. • Gathers information regarding patient's social, psychological, cultural, environmental and financial situation using this for a psychological assessment and treatment plan. • Offers assistance and concrete services, including transportation, DME, caregivers, etc. • Attends meetings, as appropriate. • Develops indicators and designs tools for tracking results. • Keeps Quality Assurance data updated. • Practices the goals and objectives of the Hospital in providing quality service and care to patients, visitors, referral services, and employees in a professional and pleasant work environment. • Adheres to the confidentiality standards of the facility. • Attends a reasonable number of in-services/continuing education programs annually. • Attends workshops offered by the State Organization of Health Care Social Workers or professional counselors. • Provides training and supervision to MSW interns following the guidelines provided by the college/university. QUALIFICATIONS • An appropriate combination of education, training, course work and experience may qualify an applicant to demonstrate required knowledge, skills, and abilities. • Master’s degree in counseling, Social Work, or a related field. • Active and unrestricted LPC or LCSW license in PA. • Two years' experience in a health care delivery system is required, including experience working with Mental Health patients. • Thorough knowledge of physiologic and psychosocial aspects of illness and the resultant relationship on the patient’s discharge plan based on that assessment, incorporating the patient and/or family’s preferences. Ability to implement patient teaching when necessary. Working knowledge of social and health resources of the community. • Excellent interpersonal, communication, and organizational skills.
