PATIENT CARE COORD

We are seeking a dedicated Patient Care Coordinator to join Stanford Health Care. This part-time position plays a vital role in our healthcare environment, ensuring patients receive excellent care and support throughout their healthcare journey. Key Responsibilities • Assist patients with scheduling appointments, ensuring an efficient flow of visits. • Serve as a primary point of contact for patient inquiries and concerns, providing timely and accurate information. • Coordinate with healthcare providers to ensure seamless communication and collaboration regarding patient care. • Maintain accurate and updated patient records in accordance with healthcare regulations. • Monitor patient follow-up care and support to ensure optimal health outcomes. • Engage in ongoing communication with insurance providers to facilitate patient coverage and billing processes. • Contribute to patient satisfaction improvement initiatives through feedback and service excellence. What We Required • Associate's Degree in healthcare administration, nursing, or related field preferred. • Excellent communication skills, both verbal and written. • Proficient in using electronic health record (EHR) software and standard office tools. • A minimum of 1-2 years of experience in a healthcare setting, particularly in patient coordination or support roles. • Certification in healthcare coordination or patient advocacy is a plus. This role is positioned within a supportive team that values collaboration. You will work closely with healthcare providers, administrative staff, and other coordinators to enhance the patient experience. Team leaders will provide guidance, and you will report directly to the Office Manager, fostering an environment of open communication and shared goals. Collaboration across various departments is essential, as you will interact regularly with billing, clinical, and management teams to ensure a holistic approach to patient care.
