This position is open ONLY to Colorado State Residents About the Work Unit This Unit is responsible and accountable for the ongoing maintenance of all accurate, complete, detailed, and confidential official legal records (paper files and electronic records) for individuals receiving services at the Pueblo Regional Center. These official records are available to authorized persons, medical staff, Pharmacy, interdisciplinary teams, staff, guardians (upon request), and community governing agencies for inspection and review. The accurate contents are a critical basis for decisions made impacting the lives and health of individuals receiving services. About the Position: This position is responsible for medical records uploading and filing in active and purged medical records, including preparing medical records for new admissions and processing records of discharged residents. This position maintains the quality of the agency's records classification system and release of medical information which includes determining record's operating procedures and monitoring the quality of the agency record system. This position serves as a resource for staff regarding issues related to medical records; ensuring HIPAA compliance. This position is responsible for researching problems/solutions, troubleshooting complaints or discrepancies, and assuring compliance with the Health Insurance Portability and Accountability Act (HIPAA) requirements, security and universal records standards as they apply to record-keeping and applicable rules, laws, and regulations pertaining either directly or indirectly to the Quality Assurance Department. This position provides backup assistance for the Quality Assurance Auditor for Residential Service records. Job duties in this position include: Medical Record Filling: • completes medical records filing in the active charts within the EHR as well as the administrative/purged files located in storage cabinets. Medical records include but are not limited to laboratory reports, radiology reports, EKG's, medical documents such as short-term protocols and emergency medication administration forms, emergency room reports, hospital reports, and a multitude of medical documents * works independently to identify, research, collect and verify accuracy and source(s) of data to ensure documents are records approved for filing • maintains the medical record content list in accordance with medical needs and standards of practice for medical record's services that are in compliance with governing agencies such as I State Health Department rules and regulations, and the Health Insurance Portability and Accountability Act (HIPAA) Records Liaison • prepares Record documents and provides requested documents to Guardians, Attorneys, and Authorized Representatives as requested are in compliance with HIPAA and governing rules and regulations • sends agency informational letters as requested by PRC management to guardians and representatives as requested • gathers, copies, sends, picks up and/or delivers requested resident information such as Incident Reports and medical consultations to guardians and representatives as requested • gathers all requested documents for State Health Audits • acts as a primary resource for records related questions and concerns Medical Record Maintenance: • maintains the records by assuring security of records and documents, replacing, repairing, retrieving, organizing, re-labeling, and completing retention and destruction functions necessary to protect highly sensitive and confidential information/files • assembles and prepares medical records for new admissions and dismantles records for individuals who are discharged and processes documents and charts for filing and storage • maintain terminated records and terminated record files, including destruction of files • conducts audits for the Quality Assurance Performance Improvement (QAPI) committee and presents the data to the monthly QAPI meetings Audits include but are not limited to verifying that guardian information is entered accurately in the medical record and electronic records and ensuring consents are current (signed by current guardian), checking select forms as directed to ensure accurate health services data has been entered, and checking to ensure our financial data such as Medicaid and Medicare numbers match Regulatory Compliance: • maintains the quality of the agency's records classification system and release of medical information that includes determining the record's operating procedures and monitoring the quality of the agency record system • serves as a resource person for agency staff pertaining to records keeping and census • Assures record unit compliance with Universal Record Standards and guidelines by independently auditing, analyzing, proofreading, monitoring, and troubleshooting discrepancies with staff/medical staff and administrators, documenting follow-up on discrepancies in record documentation for any/all client records within assigned caseload • assures compliance with confidentiality HIPAA requirements, security, and medical standards as they apply to record keeping, and applicable rules, laws, and regulations pertaining either directly or indirectly to the Records Unit • consistently monitors records, confidentiality/security, census processes and systems to assure compliance with agency needs and medical records standards • recommends procedure revisions to ensure standards for the work unit • responsible for researching problems/solutions, and troubleshooting complaints or discrepancies. Document Distribution/ Other Duties as Assigned • collects, files, and tracks documents resulting from outside facility consultations and hospital visits • retrieves requested record documents such as but not limited to Birth and Death Certificates, insurance cards, and identifying information • provides assistance to residential QA Auditor as needed for Interdisciplinary Plan related filing • provides information gathering for State Auditors as requested • copies record information for new admissions for information dissemination. Minimum Qualifications: Two years (2) of experience in medical records management Certification related to the work assigned to the position will substitute for the required experience. Substitutions: Appropriate education will substitute for the required experience on a year-for-year basis. Preferred Qualifications: • HIMSS certification • good work history • excellent customer service skills • excellent communication skills • previous experience utilizing an electronic health record • experience and training in HIPAA compliance and security • experience utilizing Google products; i.e., Google Docs, Google Sheets, Google Forms, etc Conditions of Employment: • Background Check- ALL CDHS employees (non-direct contact): CBI name check, ICON Colorado court database, Medicare fraud database, Reference checks, Professional License verification (licensure requirements), and drug screen (commercial Driver's license) * Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position.
