Provide individualized case management services to members and their families, with a focus on mental health and substance abuse. Collaborate with other healthcare professionals and community resources to ensure comprehensive care for members. Conduct assessments to determine the needs and goals of each member, and develop personalized care plans accordingly. Monitor and track progress of members, and make adjustments to care plans as needed. Act as an advocate for members, ensuring their needs are met and their voices are heard in the decision-making process. Educate members and their families on available resources and support services to promote self-sufficiency. Maintain accurate and up-to-date documentation of all interactions and services provided. Stay current on industry trends and best practices in case management and mental health/substance abuse treatment. Act as a liaison between members, their families, and healthcare providers to facilitate effective communication and coordination of care. Comply with all company and regulatory policies and procedures. Attend and actively participate in team meetings and trainings. Demonstrate empathy and understanding towards members and their families, and maintain a non-judgmental attitude. Uphold the mission and values of Aetna, and act as a positive representative of the company. Maintain confidentiality and adhere to all HIPAA regulations.
