Job Summary As a Field Community Health Worker (CHW) at UnitedHealthcare, you will play a vital role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. You will act as a liaison between our Medicaid members and the healthcare system, ensuring that they receive the care and support they need. Key Responsibilities Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care Utilize both company and community-based resources to establish a safe and effective case management plan for members Collaborate with patients, families, and healthcare providers to ensure that members receive coordinated care and services Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services Communicate with all stakeholders to ensure that health-related information is shared and that quality coordinated care and services are provided expeditiously to all members Partner with care teams, including community providers and internal staff, to ensure that members receive comprehensive care Advocate for patients and families as needed to ensure that their needs and choices are fully represented and supported by the healthcare team Document all member assessments, care plans, and referrals provided Engage members either face-to-face or telephonically, and help them set person-centered SMART goals and develop a care plan to achieve those goals Adhere to detailed, specific documentation requirements in the member's health record Proactively engage members to manage their own health and healthcare using Motivational Interviewing Skills Provide member education on community resources and benefits Requirements and Qualifications Essential Qualifications: 4+ years of case management, community outreach work, or volunteering experience 2+ years of experience working or volunteering within the Community Health setting in a field-based healthcare role 2+ years of experience working or volunteering within the local Community, with demonstrated knowledge of culture and values within the Community and familiarity with the resources Intermediate level of computer proficiency, including the use of Microsoft Outlook, Teams, and multiple web applications Must reside within a commutable distance to Kansas City & St. Joseph MO, and surrounding communities in Missouri Valid Missouri or Kansas driver's license and current automobile insurance with access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area Must reside in the state of Missouri or less than 50 miles commutable distance to Kansas City, KS Ability to travel locally, up to 100 miles round trip and up to 50% of the time Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service Preferred Qualifications: Bachelor's Degree (or higher) in a health-related field Community Health Worker (CHW) Accreditation Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant Care management experience Experience working in Managed Care Knowledge of Medicaid/Medicare population Knowledge and/or experience with behavioral health or substance use disorders Soft Skills and Competencies To succeed as a Field Community Health Worker at UnitedHealthcare, you will need to possess the following soft skills and competencies: Strong communication and customer service skills, both in person and via phone Ability to work independently and maintain good judgment and accountability Demonstrated ability to work well with others Strong organizational and time management skills Ability to multi-task and prioritize tasks to meet all deadlines Ability to work well under pressure in a fast-paced environment Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others What We Offer At UnitedHealthcare, we're committed to providing our employees with a comprehensive benefits package and a supportive work environment.
