Registered Nurse (RN) for Pediatric Home Health Care: Job Overview We're seeking an experienced and compassionate Registered Nurse to join our skilled nursing team, providing high-quality home health care to pediatric patients in the Charlottesville, VA area. As a Pediatric Home Health RN, you'll play a vital role in assessing, planning, and delivering skilled nursing care to our patients, promoting optimal health outcomes and enhancing their quality of life. Key Responsibilities Conduct comprehensive home health assessments on pediatric patients, from newborns to teenagers, to identify their unique needs and develop individualized care plans. Perform skilled nursing visits, focusing on teaching, monitoring compliance, and administering various treatments, such as NG or GTube feedings and ostomy care. Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and optimal patient outcomes. Maintain accurate and timely electronic charting using Kantime, ensuring compliance with regulatory requirements and company standards. Participate in ongoing education and clinical training opportunities to stay current with best practices, enhance your skills, and advance your career. Essential Qualifications Minimum 1 year of recent clinical nursing practice, preferably in ICU or pediatrics, with a strong passion for working with children. Current and unencumbered state licensure as a Registered Nurse. Prior Home Health experience, with a demonstrated ability to work independently and as part of a multidisciplinary team. Strong clinical skills, with experience in NG or GTube feedings and ostomy care. Preferred Qualifications Experience working in pediatric home health care, with a deep understanding of the unique needs and challenges associated with this population. Strong communication and interpersonal skills, with the ability to build trust and rapport with patients, families, and healthcare professionals. Flexibility and adaptability, with a willingness to work varied schedules and adjust to changing patient needs.
