Job Overview and Responsibilities This 13-week premium assignment offers a unique opportunity for a Registered Nurse to work in a community-based care setting, providing full-time care during the day. Prior experience in home health is required, and the successful candidate will be responsible for a range of duties, including: Conducting patient comprehensive assessments and collaborating with multidisciplinary care teams to develop and implement individualized plans of care. Supervising a patient caseload, including LPNs and HHAs, and facilitating interdisciplinary communication and follow-up through care coordination. Making referrals to other disciplines as indicated by patient needs and participating in clinical outcomes monitoring, follow-up, and care center performance improvement initiatives. Initiating, planning, and providing staff education, performing competency and skills validation for staff as needed, and utilizing accepted company teaching materials in patient/family education. Regularly communicating patient progress to the clinical manager and completing documentation timely, accurately, and at the point of care, according to industry standards and guidelines. Essential Qualifications and Skills To be considered for this role, candidates must possess the following essential qualifications and skills: One year of nursing experience, with prior experience in home health care preferred. Current RN license, specific to the state(s) of assignment, and current CPR certification. Valid driver's license, reliable transportation, and liability insurance. Strong communication and interpersonal skills, with the ability to work effectively in a team environment and facilitate interdisciplinary communication. Ability to utilize technology, including electronic medical records, and perform documentation accurately and efficiently. Preferred Qualifications and Competencies While not essential, the following qualifications and competencies are preferred: Experience working in a community-based care setting, with a focus on patient-centered care and compassion. Strong leadership and supervisory skills, with the ability to motivate and educate staff. Knowledge of industry standards, conventions, and guidelines, including OASIS assessments and care coordination. Ability to work independently, with a high degree of autonomy, and prioritize tasks effectively to meet patient needs and deadlines.
