Job Summary We're seeking a highly skilled and compassionate Registered Nurse - Clinical Care Coordinator to work with our team in Grand Junction, CO. As a Clinical Care Coordinator, you'll serve as the primary care manager for a panel of members with complex medical and behavioral needs. You'll work closely with internal care teams, providers, and community resources to develop and implement person-centered care plans, promote health and wellness, and support member self-management. Key Responsibilities Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs Develop and implement person-centered care plans to address needs, including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management, and member safety Partner and collaborate with internal care teams, providers, and community resources/partners to implement care plans Provide education and coaching to support member self-management of care needs and lifestyle changes to promote health Support proactive discharge planning and manage/coordinate care transitions following ER visits, inpatient or Skilled Nursing Facility (SNF) admissions Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the healthcare team Requirements and Qualifications Essential Qualifications Current, unrestricted independent licensure as a Registered Nurse in Colorado 2+ years of clinical experience 1+ year of experience with MS Office, including Word, Excel, and Outlook Access to reliable transportation and the ability to travel, up to 50% of the time, within Colorado (preferably the Western Slope area) to meet with members and providers Preferred Qualifications BSN, Master's degree, or higher in a clinical field CCM certification 1+ year of community case management experience coordinating care for individuals with complex needs Behavioral health experience Experience working in team-based care Background in Managed Care Skills and Competencies To succeed in this role, you'll need: Excellent communication and interpersonal skills Strong clinical knowledge and critical thinking skills Ability to work independently and as part of a team Strong organizational and time management skills Ability to prioritize and manage multiple tasks and responsibilities Compassion, empathy, and a patient-centered approach to care Career Growth Opportunities and Learning Benefits At UnitedHealthcare, we're committed to supporting the growth and development of our employees.
