Job Summary We're seeking an experienced Full Time Bilingual Medical Assistant to join our team, working remotely to coordinate patient care and support our clinical team. As a key member of our care coordination team, you'll be responsible for conducting telephonic outreach calls to patients, caregivers, and other care team members, ensuring compliance with care plans and documenting interactions in our platform. Key Responsibilities Conduct periodic telephonic outreach calls to patients, caregivers, and care team members to coordinate care and ensure compliance with care plans Collaborate with the clinical team to improve patient health and care outcomes Provide practice support, including contacting patients, caregivers, and care team members as directed Coordinate care for patients enrolled in Medicare's chronic care management program Perform data entry within operating dashboards, reporting, and workflow platforms Ensure data quality and accuracy Provide other administrative support as needed Requirements and Qualifications To be successful in this role, you'll need: Fluency in English and Spanish/Mandarin/Cantonese/Russian (writing, reading, and speaking) A minimum of 2 years of experience working in a healthcare setting Knowledge and understanding of medical terminology Knowledge and understanding of chronic care management processes A customer service mindset for both internal and external customers Medical Assistant and/or Medical Scribe Certification Strong proficiency in computer software navigation, data entry, and data cleansing Fundamental knowledge of Google Docs, Sheets, Slides, or similar Demonstrated ability to work effectively as a member of an interdisciplinary team, displaying good judgment and decision-making skills Ability to perform duties as assigned or requested Preferred Qualifications We'd love to see: Experience working with electronic health records (EHRs) and practice management systems Knowledge of healthcare regulations and compliance Certification in care coordination or case management Experience working with diverse patient populations and caregivers What We Offer At Vesta Healthcare, we're committed to supporting our team members' growth and well-being.
