Your primary responsibility will be to lead efforts to educate and engage patients in their healthcare, ensuring they have a clear understanding of the benefits and services available to them under their government healthcare plan. You will work closely with our Government healthcare team to move and motivate patients towards healthy behaviors through creative, compassionate, and empathetic interactions. **Key Responsibilities** * Collaborate with patients through phone and digital interactions to educate them about available healthcare services and benefits * Conduct inbound and outbound calls to patients who may benefit from assigned services, including medical visits, screenings, vaccinations, or medication refills * Utilize persuasive communication skills to address obstacles and gather information, providing solutions to close care gaps and delivering exceptional support to patients * Ensure that every patient is treated with respect, professionalism, and dignity, with all questions thoroughly addressed or triaged and responded to * Work as needed to coordinate care between healthcare providers, specialists, and pharmacies * Effectively listen and gather patient-reported information components in the CRM tool to support clinical record pursuit for HEDIS clinical record documentation of services * Provide patients with accurate information at the right time to help them make informed decisions about their healthcare, improving patient engagement, retention, and growth through expertly delivering coordination services to patients and providers * Connect with, counsel, and educate patients based on their unique needs, preferences, and understanding of services * Take ownership of patient insight and provide exceptional support * Respond to questions and resolve issues as a single point of contact, escalating to leadership as needed * Build trust relationships with patients by taking responsibility to understand their needs completely * Guide patients through programs, Aetna tools, and resources to support healthcare navigation * Take ownership of assigned patients to resolve their issues and connect them with additional services as needed * Report and track all patient contacts, events, and results through suitable systems and processes * Utilize professional communication skills to build relationships with both internal and external stakeholders * Leverage relevant system tools and resources to deliver high-quality business communications, including letters and accounting sheets in response to requests received * Handle multiple tasks and responsibilities while maintaining and exceeding performance standards * Recognize issues that require escalation and provide suggestions for resolution * Demonstrate exceptional skill and present a positive image of the organization while interacting with patients and constituents * Uphold individual, team, and business objectives and initiatives; take ownership of individual outcomes **Additional Responsibilities and Requirements** * Windows-based application experience with the ability to use standard corporate software packages and corporate applications with a high level of computer literacy * Effective communication skills and experience talking with clinical providers and staff * Strong listening and relationship-building skills; ability to create and maintain effective working relationships * Strong analytical skills focusing on accuracy and attention to detail * Demonstrated ability to de-escalate situations * Demonstrated empathy, interest, enthusiasm for learning, empathy, and listening skills * Ability to apply creative thinking skills when needed to address patient interests and a willingness to advocate for them * Ability to adapt to change, adjust to different priorities in short periods of time * Ability to work independently, apply decisive reasoning skills when needed, perform multiple tasks easily, and exhibit exceptional skill in all interactions * Ability to resolve complex issues with responsiveness and caution * Ability to absorb and apply new and evolving information * Ability to make successful and autonomous decisions * Ability to perform multiple tasks, focus, and really adapt to a high-speed changing environment **Required Qualifications** * Recent and relevant experience working with HEDIS screening measures and behavior change * Related knowledge working directly with patients/consumers – ideally in a call center setting * Minimum 2 years of ongoing and related experience within healthcare, healthcare marketing, outreach, as well as health education, health instruction, and disease management experience * 2-4 years of experience in a healthcare setting with direct communication with various levels of management, including clinical providers **Preferred Qualifications** * Bilingual or multilingual candidates are highly preferred * Four-year degree or equivalent is preferred * Clinical certification (certified nursing assistant, clinical assistant, etc.) is strongly suggested but not required **Education and Accreditation Requirements** * High School Diploma or G.E.D. **Compensation and Benefits** The typical pay range for this position is: This pay range represents the base hourly rate or base yearly full-time compensation for all positions in the job grade within which this position falls. The actual base compensation offer will depend on various factors, including experience, education, location, and other relevant variables.
