Job Responsibilities The successful candidate will be responsible for performing patient-centered clinical administration programs for both payer and pharma partners to ensure total and accurate patient information is captured. This will involve working with various stakeholders, including doctors, professionals, and office clinical staff, to provide high-quality patient care services. Key Responsibilities: Perform initial evaluations for patients, which may include helping with mixture pump programming and investigating based on Plan of Treatment (POT) Conduct framework-driven evaluations, such as Misery Screenings for select disease states Conduct follow-up evaluations and efforts to determine medication history and to improve medication adherence Provide disease state education, including medication organization, resources on the management of side effects, etc. Be accountable for the day-to-day nursing care activities, including the completion of nursing coordination/plan of treatment forms, building supply records, nursing evaluations, initial screenings, and reviews for patients Review and report daily in group and individual performance metrics and quality insights and assist with the extrapolation of framework applications to develop reports of information regarding efficiency patterns Assist with creating, updating, and implementing departmental Standard Working Procedure related to framework applications, projects, processes, and products Provide support within the scope of training for a nurse and escalating to a pharmacist for patient-specific medication questions, including the management of side effects and patient satisfaction screening May work with specific nursing specialties, such as but not limited to Mixture Care, Richness Request, Nursing Organization, and Care Management Requirements To be successful in this role, you will need to have the following qualifications and skills: Essential Qualifications: Partner's certification in Nursing Current license/enrollment as a RN (Enlisted Nurse) with the State Board of Nursing in the state in which the position resides Should acquire and maintain Medical Assistant Permitting Minimal multi-state permit and fundamental non-nursing Authorizing Smaller state permit in no less than a half year of work start date (organization supported); all exceptions for this timeframe require management endorsement Preferred Qualifications: Four-year college education in Nursing Dynamic Nursing Authorizing Conservative multistate permit and nursing license(s) in states not participating in Nursing Licensure Minimized Experience working with contracting and agreement requirements is vital for the suitable strength Skills and Competencies The successful candidate will need to possess the following skills and competencies: Essential Skills: Ability to work in a group climate Coordinated and conscientious with demonstrated critical thinking abilities Superb written/verbal communication and interpersonal skills with the ability to lead, motivate, and communicate with internal and external clients Knowledge of client care standards and handling including client needs assessment, fulfilling quality standards for services, and assessment of customer satisfaction Essential skills in MS Office Suite Career Growth Opportunities and Learning Benefits At Walgreens, we are committed to providing our employees with opportunities for career growth and development.
