Overview We're seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Home Health Services Nurse. As a key member of our team, you'll be responsible for coordinating and overseeing all aspects of patient care, collaborating with patient care teams to develop and implement care plans, and educating families on patient condition, needs, and care. Responsibilities As a Home Health Services Nurse, you'll be responsible for: * Coordinating and overseeing all aspects of patient care * Collaborating with patient care teams to develop and implement care plans * Educating families on patient condition, needs, and care * Performing head-to-toe physical assessments * Checking vital signs and managing medications * Giving injections, IV therapy, or tube feedings * Administering and maintaining catheters * Completing documentation timely and accurately * Supervising LPNs and HHAs * Regularly communicating patient progress to the clinical manager, physician, and care team * Other duties as assigned Qualifications To be considered for this exciting opportunity, you'll need to meet the following qualifications: * One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical. * Current RN license, specific to the state(s) you are assigned to work. * Minimum six months home health experience. * Current CPR certification. * Valid driver's license, reliable transportation, and liability insurance. * Ability to assess patient status and identify requirements relative to age-specific needs. * Familiarity with general use and functions of the computer, such as user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs. * Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
