About the Role In this essential role, you will be an integral part of our Integrated Care Model, working closely with our members, healthcare providers, and community resources to ensure that our members receive the best possible care. As a Field RN Care Coordinator, you will assess, plan, and implement care strategies that are individualized to each member's needs, advocating for their rights and ensuring that they have access to the services and support they require. You will work collaboratively with our team of RNs, LVNs, and CACs to help our members achieve their healthcare goals and improve their overall well-being. Key Responsibilities Assess, plan, and implement care strategies that are individualized to each member's needs, directed towards the most appropriate, least restrictive level of care Identify and initiate referrals for member transition to community request and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services Manage the care plan throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members Advocate for members and families as needed to ensure that the member's needs and choices are fully represented and supported by the healthcare team Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices, to provide support and guidance Requirements and Qualifications To be successful in this role, you will need to possess a strong foundation in nursing, as well as excellent communication and interpersonal skills. You should be comfortable working in a fast-paced environment, prioritizing multiple tasks and responsibilities, and maintaining a high level of organization and attention to detail. Essential Qualifications Current and unrestricted RN license in the state of Texas 2+ years of experience working within the community health setting or in a healthcare role Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities, or providers' offices Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) Reside in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service Preferred Qualifications 1+ years of experience with long-term care services and support, Medicaid, or Medicare Ability to create, edit, save, and send documents, spreadsheets, and emails Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations Reside within commutable distance of assigned duties Career Growth and Development At UnitedHealthcare, we are committed to helping our employees grow and develop in their careers.
