Case Manager RN- Field (Southwest Michigan)
About the position Responsibilities • Assess and evaluate the medical needs of members to facilitate their overall wellness. • Plan and implement case management activities in collaboration with members. • Coordinate care and services for members, ensuring they receive appropriate support. • Act as a frontline advocate for members who cannot advocate for themselves. • Travel within the designated coverage area to meet with members as needed. Requirements • Active and unrestricted RN licensure in the state of Michigan. • Minimum of 3 years of clinical practical experience. • Minimum of 2 years of experience in Care Management, discharge planning, or home health care coordination. • Reliable transportation for travel within the coverage area. Nice-to-haves • Certified Case Manager certification. • Confidence working independently and using virtual collaboration tools. • Excellent analytical and problem-solving skills. • Effective communication, organizational, and interpersonal skills. • Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint.
