Case Manager – New Choices Waiver (Management Track)
About the Role Beehive Community Case Management is growing and seeking a motivated SSW Case Manager to support Medicaid New Choices Waiver (NCW) members throughout Northern Utah. This is a hybrid position with flexibility, autonomy, and a clear path to leadership. What You’ll Do • Complete NCW assessments, Person-Centered Care Plans, and ongoing case management • Conduct facility and home visits (AL, MC, SNF, private home clients) • Coordinate services with providers, families, facilities, and state partners • Ensure compliance with NCW guidelines and documentation requirements • Maintain accurate, timely, person-centered records • Growth track: participate in leadership development and eventually supervise case managers Compensation • $32 per billable hour (paid in 15-minute billable units) • Flexible schedule — autonomy in organizing your caseload • Leadership bonuses and advancement opportunities • Future eligibility for profit sharing or ownership-track roles Ideal Candidate • Active Utah SSW license (Required) • Strong organization and communication skills • Comfortable working independently in the field • Able to complete timely documentation • Passion for working with seniors and vulnerable populations Preferred • Experience with New Choices Waiver • Background in assisted living, home health, hospice, SNF, or case management Why Join Beehive CCM? • Work directly with founders who value input & innovation • No micromanaging — complete autonomy in your workflow • Supportive small-team environment • Real career growth, not a dead-end case management job • Huge long-term potential as we expand across Utah Location Hybrid role: Remote documentation + field visits in Weber, Davis, and Salt Lake counties.
