Bilingual Nurse (RN) with experience in Primary Care, Geriatrics and Palliative Care in GA and CA
Job Summary We are seeking a compassionate and dedicated Registered Registered Nurse with interest in or with a specialty in Geriatrics and Palliative Care to join our healthcare team. We are a small and growing home-based practice that serves patients in Northern California and regions surrounding Atlanta, Georgia. The ideal candidate is licensed in both states, or willing to add license to both states, is English and Spanish speaking, and has excellent communication and triage skills. This role requires excellent clinical skills, empathy, and the ability to work collaboratively with a multidisciplinary team to support patients and their families. This role is remote and involves helping the physicians and APPs respond telephonically or virtually to patient needs by helping to triage. The role is part time with the option to expand to full time with benefits if right fit and as practice grows. Need minimum of 3 days a week (8-12, or 1-5), some weekend work may be OK but most the work must be during daytime business day hours. Must be accessible by phone during assigned work hours. Duties • Develop and implement individualized care plans in collaboration with the healthcare team. • Provide education and support to patients and families regarding disease processes, treatment options, and palliative care resources. • Assist with triaging of symptoms and following symptom based protocols to help patients stay in their homes when within their goals • Have an interest in serving people living with Dementia and helping families navigate difficult symptoms. • Assist in managing symptoms related to serious illnesses, including pain management strategies. • Collaborate with other healthcare professionals in long-term care settings to ensure continuity of care. • Maintain accurate medical records using Athena and ensure compliance with ICD-10 coding standards. Qualifications • Current Registered Nurse (RN) license in the state of practice. • Previous experience in Geriatrics, palliative care or related fields is preferred. • Strong knowledge of medical records management and familiarity with electronic medical record documentation. • Proficiency in ICD-10 coding for accurate documentation. • Excellent communication skills, both verbal and written, with the ability to empathize with patients and families during challenging times. • Must be able to work effectively within a multidisciplinary team environment. • Must be willing to work remotely. • Preference is to bilingual (Spanish and English) applicants Job Type: Part-time Pay: From $44.00 per hour Benefits: • Paid time off Experience: • Geriatrics: 1 year (Preferred) Shift availability: • Day Shift (Required) Work Location: Remote Apply tot his job
