AGING LIFE CARE MANAGER ROLE Aging Well Advocates, a private Aging Life Care Management practice, is looking to hire an empathetic and dynamic care manager who loves working with the aging population to provide expert guidance and consultation for seniors and their families in making complex healthcare and long-term care decisions. The care manager will create individualized care plans for patients, collaborating with all healthcare providers, as well as the patients' families, friends, and caretakers to obtain critical information relevant to the patient’s unique situation and needs. The ideal candidate will be skilled at identifying customized solutions and resources carefully selected for each client, based on comprehensive assessment findings. The care manager will excel at continually developing a network of quality providers throughout the healthcare system, maintaining accurate patient records, and ensuring patients are linked to the most appropriate programs and resources. The world of private care management is different each day. The person most successful in this role will thrive on flexibility, creating your own schedule, and being adept at self-organization. Time management, ability to multi-task, and effective communication skills are a must. A proactive approach to helping clients navigate the complicated landscape of aging is one of the most crucial aspects of care management. Care Manager Responsibilities: • Create and update written summaries of findings and recommended personalized care plans for patients and responsible parties. • Meet and collaborate with patients' families, friends, and social supports to gather information and assess the process of developing treatment plans. Assessments to include medical, cognitive, emotional, functional, and safety components to afford recommended resources. • Implement and monitor all aspects of the care plan, including follow through on action items, identifying obstacles, and updating/modifying goals as needed. • Navigate complex family and professinal dynamics to overcome challenges. • Identify and recruit high-quality healthcare providers and facilitate patients' treatments. • Follow up with providers to ensure patients’ treatment requirements are met. • Recommend alternative treatment plans when patients' services do not meet medical necessity criteria. • Instruct and educate patients on procedures, healthcare provider instructions, and referrals. • Careful documentation of services provided in a timely and thorough manner. • Link patients to services programs/entitlements such as transportation assistance and translation services. • Conduct regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes. • Maintain comprehensive records of case management activities. • Educate professionals and families regarding the Aging Well Advocates business and how our services can help clients/family members. Knowledge, Skills, and Abilities: • Genuine interest in helping seniors. • Integrity, honesty, and commitment to high standards of excellence and professional ethics • Knowledge and experience with the aging process, cognitive changes and other conditions affecting older adults. • Highly skilled in formulating holistic assessments and developing care plans that incorporate effective use of resources, creative problem solving, and sound care management judgment. • Excellent interpersonal skills and ability to connect with a wide range of individuals, including physicians, and legal and financial professionals. • Knowledge of the aging community including levels of care, cost, state regulations, and insurance parameters. • Strong analytical and problem-solving skills, as well as verbal and written communication abilities. • Impeccable attention to detail and follow through. Qualifications: • Bachelor’s degree in social sciences, social work, or nursing. • 2+ years of work experience in healthcare settings. • 1+ years of current case management experience. • Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure. • Licensed Independent Clinical Social Worker (LICSW)--license preferred. • Proficient with Healthcare Management Systems and Microsoft Office Word and Excel. • Outstanding communication skills, both written and verbal. • Excellent organizational skills. • Ability to travel locally.
